HS Winter Sport Registration

OKSD Athletics has partnered with FinalForms, an online forms and data management company. FinalForms allows you to complete and sign required athletic participation forms. FinalForms saves data from season to season and year to year meaning that you will never have to enter the same information twice. Requirements for OKHS Athletic Participation

1. Register at Final Forms. All registration forms must be completed in order for you student-athlete to be eligible to practice.

2. Submit a Sports Physical (Physicals are valid for 24 months)

3. Prior to the first contests an ASB card must be purchased. A high school ASB card is $45. ASB cards allow student athletes free admission to all OKHS/MS regular season contests. and only need to be purchased once per school year.

Winter Sport practices begin on November 17th.
