Students with potentially life-threatening health conditions

Students with potentially life-threatening health conditions (asthma, seizures, diabetes, severe allergy) require an Emergency Care Plan (ECP) to be in place before students attend school. Any medications that are prescribed for a life threatening condition are REQUIRED to be in place before the student attends school. A medication authorization from your health care provider must be completed also.  Please contact your school nurse if you have questions. Medication authorizations are located in the FORMS sections of this webpage.
